Plan Overview
  A&I administers the Select and Choice Medical Plans prior to January 1, 2008. We have also provided overviews of the Rx, Dental and Vision Plans as well.  You can view highlights of these Plans by clicking on the tabs below.
 
 
 
  Dental Plans
 

The following Dental Plan options are provided by the Tuality Healthcare Benefits program and are administered by ODS.  The information provided here is only a summary.  Please refer to your dental benefit booklet for details on your dental plan coverage's or contact ODS directly.
 
 
 
  Preventive Plan
 

Under this plan, you can choose to see any licensed dental provider.  After you pay a $50 per person annual deductible, the plan will pay a percentage (coinsurance) of covered expenses, up to $1,500 per person per year.  There is no deductible required for diagnostic/preventive care. 
 
 

Under both plans, you may be subject to additional fees if services are received from non ODS participating dental providers.

Waiving coverage. You may waive dental coverage for you and/or your dependents.  However, you will need to have a routine exam with x-rays and have any needed corrections done before being able to enroll for these benefits at a future time.
